**Direct Support Professional – Mid Cape: $15.00 to $17.00 based on experience** – $500 Sign-On Bonus! – Part Time & Full Time
The Direct Support Professional provides instruction, support, and serves as a positive role model to individuals with developmental and intellectual disabilities, autism, acquired brain injury or other unique challenges. DSPs are expected to encourage development of life, social and recreational skills, community and health care access.
High school diploma or equivalent required. 1 year of direct care, CNA, or human services experience preferred. Ability and willingness to provide hands-on personal care. Must be able to effectively communicate in both verbal and written form with coworkers and supported individuals and read fluently. Valid driver’s license in the state of Massachusetts required.

Assistant Residential Program Director – Full Time – Hyannis
The Assistant Residential Program Director is responsible for the successful daily operations of our comprehensive Residential Supports creating a safe and therapeutic environment for the people served. The Assistant Program Director will focus on coordinating the onboarding and supervision of a Residential Management Team as well as a DSP team.
Bachelor’s degree preferred. Minimum 6 years prior experience including 3 years residential management and staff management experience and 3 years supporting individuals with intellectual and developmental disabilities, autism, brain injuries or other unique challenges. Knowledge of regulatory compliance. Prior supervisory experience required. Ability and willingness to provide hands-on personal care. Strong organizational skills. Must be able to effectively communicate in both verbal and written form with coworkers and supported individuals and read fluently. Valid driver’s license in the state of Massachusetts required. Successful completion of all state and Agency mandated trainings required upon hire. Successful completion of Medication Administration Training certification required upon hire.

Residential Program Manager – Full Time 40 Hours
The Residential Program Director is responsible for the successful daily operations of our comprehensive Residential Supports creating a safe and therapeutic environment for the people served. The Program Director will focus on coordinating the onboarding and supervision of a Residential Management Team as well as a DSP team. The Residential Program Director ensures the overall supervision, evaluation, and discipline of the entire Residential team including managers, assistant managers, DSPs, and the relief pool. Additionally, ensures each member of the Residential team is current with trainings, certifications, supervisions, and evaluations.
Bachelor’s degree preferred. Minimum 6 years prior experience including 3 years residential management and staff management experience and 3 years supporting individuals with intellectual and developmental disabilities, autism, brain injuries or other unique challenges. Provides support and assistance to program participants in the areas of independent living, self management within the community, social and independent living skills development and affective and behavioral management. Communicates with families and providers, as required.

As a member of the nursing and care management team, provides social work services that facilitate the screening of new family homes and participants; assists in matching and placement services to participants.
Bachelor’s degree. Social worker license from the Massachusetts Board of Registration in Social Work. Minimum 2 years prior experience working with elderly or disabled adults. Bachelor’s degree plus 2 years clinical experience in the care of elderly or disabled individuals may be substituted for the above requirements. Attention to detail. Valid driver’s license in the state of Massachusetts required. Successful completion of all state and Agency mandated trainings required upon hire.

Residential Healthcare Nurse Supervisor – 40 Hours per week – Monday – Friday Flex
In conjunction with residential management, provides a comprehensive system of residential services creating a safe and therapeutic environment that fosters the development of human dignity and focuses on personal well-being.
Valid Registered Nursing License in the state of Massachusetts. Minimum 2 years prior experience working in a medical setting as a registered nurse. Valid driver’s license in the state of Massachusetts. Demonstrated experience in community health nursing, visiting nursing and/or nursing specific to individuals with unique developmental or traumatic brain injury challenges. Successful completion of all state and Agency mandated trainings. Ability to complete state mandated certification program to teach coursework developed by the Department of Public Health (DPH) Medication Administration Program. Ability to travel to residential homes throughout Cape Cod to provide medical clinical assessments. Ability to provide hands-on personal care.
